recurrent infections and bilateral uveitis in a patient cd8 deficiency

چکیده
cd8 deficiency is a rare primary immunodeficiency with low or absent peripheral cd8 cells which results from tap deficiency, zap 70 deficiency and cd8 α gene mutation. we report a 14 year old female who presented with a history of recurrent pneumonia, bronchiectasis, otitis, severe varicella, herpetic lesions of mouth, bilateral uveitis, and cataract formation since the age of 8 years. she had growth failure, a huge spleen and moderate clubbing. in immunologic workup, humoral and phagocytic systems were normal .dth response to candida, ppd and dt were negative but ltt response to pha mitogen was normal. hla typing showed normal class i expression. flowcytometry of peripheral blood showed cd8: 0 to 2% (absolute count, 0-60 cells/mm3) with increased cd4/cd8 ratio on several occasions. diagnosis of this patient cannot be hla class i deficiency (tap1 or tap2), because class i expression had been normal. it is possible to be zap -70 deficiency or cd8 α gene mutation. bilateral uveitis in our patient was a unique presentation which might have resulted because of immune dysregulation in cid.

Abstract In this presentation, a case of bilateral choroidal folds in patient with diabetic retinopathy was reported. By our knowledge, this case presentation is the first report of choroidal folds in diabetic retinopathy, which bilateral symmetric and horizontal folds arranged in macula. The thick choroid that is developed in diabetics may be a cause of these folds but the source of differenc...
متن کاملIRAK4 Deficiency in a Patient with Recurrent Pneumococcal Infections: Case Report and Review of the Literature
Primary immunodeficiencies are genetic defects of the innate or adaptive immune system, resulting in a propensity to infections. The innate immune system is the first line of defense against pathogens and is critical to recognize microbes and start the inflammatory cascade. Sensing of microbes occurs by a number of pathogen-recognition receptors, resulting in the activation of inflammatory sign...
متن کاملSelective Antibody Deficiency and its Relation to the IgG2 and IgG3 Subclass Titers in Recurrent Respiratory Infections
Background: Selective antibody deficiency with normal immunoglobulins (SADNI) may be identified as part of distinct primary or secondary immunodeficiency disorders. The clinical manifestations include recurrent, often severe or prolonged, upper or lower respiratory tract infections. Objectives: To evaluate SADNI in patients with recurrent sinopulmonary infections and its relation to IgG subclas...
متن کاملRecurrent rhinovirus infections in a child with inherited MDA5 deficiency
MDA5 is a cytosolic sensor of double-stranded RNA (ds)RNA including viral byproducts and intermediates. We studied a child with life-threatening, recurrent respiratory tract infections, caused by viruses including human rhinovirus (HRV), influenza virus, and respiratory syncytial virus (RSV). We identified in her a homozygous missense mutation in IFIH1 that encodes MDA5. Mutant MDA5 was express...
متن کاملInterleukin 12 deficiency associated with recurrent infections.
A 3-yr-old female patient exhibited interleukin 12 (IL-12) deficiency that was associated with recurrent episodes of pneumococcal pneumonia with sepsis and other infections in the absence of fevers. The patient's peripheral blood mononuclear cells (PBMCs) exhibited normal proliferative responses to antigens. Immune responses, including in vivo production of antibodies to diphtheria, tetanus, or...
